மூட்டுப் பிரச்னைக்கு முற்றுப்புள்ளி!

வயதானவர்களை மட்டுமே வருத்தி வந்த மூட்டுத் தேய்மானப் பிரச்னை இப்போது இள வயதினரையும் தாக்கத் தொடங்கிவிட்டது. கை, கால், விரல்கள் உள்ளிட்ட உறுப்புகளின் அசைவுகளுக்கு காரணகர்த்தாவாக இருப்பவை, மூட்டுகள்தான். மூட்டுகளில் தேய்மானம் ஏற்படும்போது, அது தொடர்பான உறுப்பின் மொத்த இயக்கமும் பாதிப்புக்கு உள்ளாகிறது. சில சமயங்களில் நடக்கவே முடியாத அளவுக்கு ஆட்களைப் படுக்கையிலும் சாய்த்துவிடுகிறது. 

மூட்டுத் தேய்மானத்தைச் சரிசெய்ய அறுவை சிகிச்சை வரை மருத்துவம் வளர்ந்துவிட்டாலும், முழுமையான தீர்வுக்கு சாத்தியம் இல்லாத நிலை. இப்போது புதிதாக வந்திருக்கும் 'கார்டிலேஜ் டிரான்ஸ்ப்ளான்டேஷன்' சிகிச்சை மூலம் மூட்டு தேய்மானத்தை நிச்சயம் சரிசெய்யலாம் என்கிறார்கள் மருத்துவ வல்லுநர்கள். இது குறித்து எலும்பு மற்றும் மூட்டு சிகிச்சை நிபுணர் இளங்கோவனிடம் பேசினோம்.

இத்தகைய தேய்மானம் பெரும்பாலும் முழங்கால் மூட்டு பகுதியிலேயே அதிகம் ஏற்படுகிறது!'' என்றவர், அதற்கான காரணங்களையும், அதை சரிசெய்யும் கார்டிலேஜ் டிரான்ஸ்ப்ளான்டேஷன் சிகிச்சை பற்றியும் விளக்கினார்...

''பொதுவாக 20 வயதில் இருந்து 45 வயது வரை நாம் அதிகமான வேலைப் பளுவைச் சுமக்கிறோம். வண்டி ஓட ஓட சக்கரம் தேய்வதுபோல், ஒருவருடைய வேலையைப் பொறுத்து, அவருடைய மூட்டில் உள்ள கார்டிலேஜ் என்கிற பகுதியும் தேய்கிறது. அதிக நேரம் நடப்பவர்கள், நின்றுகொண்டே வேலை செய்பவர்கள், அடிக்கடி படிக்கட்டுக்களில் ஏறி இறங்குபவர்கள் மூட்டு தேய்மானத்தினால் அதிக அளவில் பாதிக்கப்படுகின்றனர். கார்டிலேஜ் பகுதி மட்டும் இன்றி அதற்கு சக்தியை அளிக்கும் சைனோவியல் ஃப்ளூயிட் என்கிற திரவம் குறைவதாலும் மூட்டுத் தேய்மானம் ஏற்படுகிறது. பொதுவாக எலும்பு உடைந்தால், மறுபடி வளரும். ஆனால், இந்த கார்டிலேஜ் பகுதி பாதிப்புக்கு உள்ளானால் மீண்டும் வளர்வது இல்லை. அதனை வளரவைத்தால், இந்த மூட்டு தேய்மானத்துக்கு நிச்சயம் தீர்வு காண முடியும் என்ற சிந்தனையில் தோன்றிய ஆராய்ச்சிதான் கார்டிலேஜ் டிரான்ஸ்ப்ளான்டேசன்!

சைனோவியல் ஃப்ளூயிட் அளவு குறைவது மூட்டு தேய்மானத்தின் ஆரம்ப கட்ட நிலை. இதைப் போக்க செயற்கையாக உருவாக்கப்பட்ட சைனோவியல் ஃப்ளூயிட்டை ஊசி மூலமாக மூட்டுக்குள் செலுத்துவார்கள். கார்டிலேஜ் பகுதியில் பாதிப்பு எனில், அதற்கு மூட்டு மாற்று அறுவை சிகிச்சைதான் சிறந்தது. இதில் செயற்கையாக மூட்டு செய்து பொருத்தப்படும். பெரும்பாலான எலும்பு முறிவு நிபுணர்கள் இந்த முறையைத்தான் கையாளுகின்றனர். அதில் இருந்து முற்றிலும் மாறுபட்டது கார்டிலேஜ் டிரான்ஸ்ப்ளான்டேஷன் சிகிச்சை. வெளிநாடுகளில் இந்த சிகிச்சை பரவலாகப் பின்பற்றப்படுகிறது. இந்தியாவில் பெரும்பான்மையான நகரங்களில் இம்முறை தற்போது கையாளப்பட்டு வருகிறது.  

இதில், இறந்தவர்களின் உடம்பில் இருந்து கார்டிலேஜ் பகுதியை எடுத்து தேய்மானத்தினால் பாதிக்கப்பட்டவர்களுக்குப் பொருத்தப்படும். 20 வயதில் இருந்து 25 வயதுக்குள் இறந்துபோனவர்களுடைய கார்டிலேஜ் பகுதிதான் இதற்குப் பொருத்தமானது. இந்த வயது உடையவர்களின் கார்டிலேஜ்  பகுதி அதிகத் தேய்மானத்துக்கு உள்ளாகாமல் இருக்கும். மூன்றில் இருந்து ஆறு மாதங்களுக்குள் இந்த கார்டிலேஜ் பகுதி ஒட்டி வளர ஆரம்பித்துவிடும். இதில் உள்ள ஒரே சிக்கல் பொருத்தப்படும் கார்டிலேஜை, அந்த நபரின் உடல் ஏற்றுக்கொள்ள வேண்டும் என்பதுதான். தற்போது இந்த முறையில் 50 பேர்களுக்கு கார்டிலேஜைப் பொருத்தினால் 30 பேருக்குத்தான் பொருந்துகிறது.

இதைத் தவிர கார்டிலேஜ் பகுதி தேய்மானத்துக்கு உள்ளானவர்களிடம் இருந்தே கார்டிலேஜை எடுத்து வெளியில் வளரவைக்கலாம். இதனால் கார்டிலேஜ் கொடுப்பவர்களுக்காக நாம் காத்திருக்க வேண்டிய அவசியம் இல்லை. ஆனால், இதற்கான தொழில்நுட்பம் தற்போது நம் நாட்டில் இல்லை. இதனால், இறந்த பிறகு கண் தானம் செய்வதுபோல் இந்த கார்டிலேஜ் பகுதியையும் தானம் செய்ய மக்கள் முன்வந்தால், இந்த சிகிச்சையை வெற்றிகரமாக செய்து முடிக்கலாம்.

இதனால், எதிர்காலத்தில் செயற்கையாக மூட்டு பொருத்துவதற்குப் பதிலாக இயற்கையாகவே இந்த பிரச்னைக்குத் தீர்வு காண முடியும். இதனால் பெரிய அளவில் செலவையும் குறைக்க முடியும்!'' என நம்பிக்கை வார்க்கிறார் டாக்டர் இளங்கோவன்.

Global Offshore Drilling Pace Continues to Gather Steam

A current global snapshot reveals that there are 760 offshore rigs divided into two main types (277 floaters and 483 jackups) within the competitive fleet. The table above details the type of rig, the water-depth accommodated, and current region where these rigs operate. The highest concentration of shallow water rigs is located in the Middle East, which hosts nearly 31 percent of the total mix or 149 jackup rigs. From a highest contracted percentage, the best performing region is the North Sea, where 93 percent or 39 of 42 jackup rigs are leased. But the most active region for jackups falls back to the Middle East where 114 rigs are now under contract.

Latin America with 80 rigs leads the pack in terms of where the most deepwater rigs reside .  Two regions, the North Sea and SE Asia, are tied for second.  Each has 41 floaters occupying their respective waters. Of the two, the North Sea is putting a greater percentage to work with 95 percent (39 of 41 floaters under contract). Australia/Asia Pacific and Latin America (predominantly Brazil) boast perfect utilization rates in their regions with 16 and 80 floaters under contract, respectively. The next table highlights specific utilization rates by type of rig and region.

Dayrates

Transocean's Deepwater Champion, an ultra-deepwater drillship, currently possesses the highest dayrate at $690 k/day and is operating in the Black Sea. The Champion's rate, along with rigs working in remote locations, skews the average for UDW floaters in the "Other" row. The average dayrate for all 248 floaters in operation is the mid-$380s. On a region-by-region basis, West Africa, the Gulf of Mexico and the North Sea are all posting strong dayrate averages for their floating fleets.

Rowan Stavanger, currently drilling in the North Sea, commands the highest dayrate for a jackup at $340k/day. Given the harsh environment and stricter standards, it is not surprising to find the highest jackup dayrates in the North Sea region at an average in the mid-$150s. The average dayrate for all 348 jackups currently under contract is the high-$100s. Besides the North Sea, rigs operating in Australia/Asia Pacific, Southeast Asia, and the Middle East are all currently posting dayrates that exceed the global average for jackups.

Looking to the Future

Beginning last fall, the offshore drilling industry kicked off a rig construction campaign that now—one year later—is approaching 100 offshore rigs on order. The delivery dates for these 52 jackups and 41 floaters are expected between July 2012 and January 2018. Given that option's outstanding number—nearly 50 rigs (40 percent floaters, 60 percent jackups)—it is easy to see why the overall rig market could top 900 units in the next five years, even with some attrition.

We continue to hear market commentary that points towards tightening rig demand. This is a definite contrast to the swelling apprehension globally regarding economic concerns. Our own internal independent forecasts for rig demand support the view of a sustained, albeit anemic, global recovery. The recent jobs data does not change our view. Overall we are anticipating that rig demand will grow 10 percent on average during 2012. Furthermore, 2013 looks to be the year where we see sustained drilling efforts using more than 600 rigs throughout the world.

Myanmar O&G Exploration Tender Draws over 50 Bids

Myanmar has received more than 50 bids for an oil and gas exploration tender, which closed last week, a government official said Thursday.

The bidders are from China, Europe, Middle East, Russia and Southeast Asia, the official, who didn't wish to be named, said.

The energy planning department is now short-listing the bidders and will invite selected firms to review the geological and geophysical data of 18 onshore blocks, mostly in central areas, the official said.

Each short-listed foreign firm is required to form a joint venture with an approved Myanmarese company to bid in the tender. Local companies participating in the tender have to register with the energy ministry by Sept. 9.

Once the review of geological and geophysical data is completed, the joint ventures can submit details on the work program and budget to the ministry.

As of April 1, 2008 the country's proven onshore and offshore crude-oil reserves were 112 million barrels and 101 million barrels respectively, Myanmar Oil and Gas Enterprise, which regulates the upstream oil and gas sector, said previously.

Proven onshore and offshore natural gas reserves totaled 0.46 trillion cubic feet and 17 trillion cubic feet respectively.

BP Moscow Office Raided As Pressure In Russia Rises

Russian court officials Wednesday raided the Moscow office of BP in the latest setback for BP in Russia following the collapse of a proposed $16 billion share-swap and Arctic oil exploration deal with state-controlled Rosneft.

The raid comes only a day after Rosneft signed a deal with ExxonMobil to explore the same offshore Arctic fields and work together in the U.S. and other locations.

Russian bailiffs entered BP's Moscow office to examine documents requested in a $3 billion lawsuit filed by Siberia-based minority shareholders in BP's exchange-listed Russian joint venture, TNK-BP Holding, BP spokesman Vladimir Buyanov said.

BP's London office said there are "no legitimate grounds for such a raid" and that the "entity raided has no connection with the process in Tyumen," Siberia, said a BP London spokeswoman. The documents seized by bailiffs "are confidential and have no connection with any shareholder issues," she said.

BP shares were flat Wednesday at 397 pence at 1123 GMT, slightly underperforming the broader U.K index. Some analysts said the Exxon-Rosneft deal was a big negative for the company, but others said the disappointing demise of the venture was already priced into the shares.

The escalating tensions are reminiscent of 2008, when BP fought a bitter battle in TNK-BP over strategic control of the venture that spawned raids by immigration officials and harmed relations between Russia and the U.K.

Dmitry Chepurenko, a lawyer representing TNK-BP minority shareholders, said BP Exploration Operating Company Ltd. didn't comply with a court order to provide documents connected with the abortive BP-Rosneft partnership. Thus the court in Tyumen, where the minority shareholders are based, Monday ordered bailiffs to retrieve the documents from BP Exploration Operating Company Ltd., Chepurenko said in a statement.

BP holds its TNK-BP stake through a unit based outside of Russia, although the staff who work with BP work out of its Russian office, which is also home to BP Exploration Operating Company Ltd. unit, BP's Buyanov said.

Most of BP's approximately 120 Moscow employees left work Wednesday as requested by the bailiffs, leaving mainly legal staff and security to work with the officials, Buyanov added. BP Russia chief Jeremy Huck also left the office and currently remains in Russia, the spokesman said.

TNK-BP minority shareholders are seeking RUB87 billion ($3 billion) from BP due to alleged losses stemming from BP's failed tie-up with Rosneft.

The powerful Alfa-Access-Renova consortium of Soviet-born businessmen successfully blocked the BP-Rosneft deal using a clause in the TNK-BP Ltd shareholder agreement.

Lawyers for the TNK-BP minority shareholders, who live in Tyumen, where TNK-BP Holding is based, said they have no connection with the consortium of billionaires, known as AAR.

BP in 2008 fought a losing battle with AAR for control of TNK-BP, then led by current BP Chief Executive Robert Dudley. The conflict included court battles in Tyumen as well as the removal of BP staff from Russia. The acrimonious dispute was resolved only when BP ceded greater influence over the joint venture to its Russian partners and Dudley resigned as TNK-BP CEO.

In a separate case from the Tyumen arbitration, AAR this month said it has asked a Stockholm arbitration panel to issue a final ruling on whether BP breached the TNK-BP shareholder agreement.
